SLIGHT INCREASES IN SERUM CREATININE LEVELS AS A PROGNOSTIC FACTOR IN CHRONIC HEART FAILURE

Abstract

Objective

The goa/ of this study was to verify the relation between slight increases in serum creatinine leveis and hospital admission and!or mortality in patients with chronic heart failure.

Methods

122 patients with chronic heart failure (67 men and 55 women; mean age of 73.9 ± 6.89 years), classified according to the New York Heart Association, were evaluated. The patients were separated into the fo/lowing groups: group A - with normal serum creatinine leveis (men::;1 .3mg%; women :,;7.0mg%) with 67 patients; and group B - with slightly a/tered leveis (men ;?1.4mg%; women;?1. 1mg% and <2. 1mg% for both genders) with 55 patients. Frequency of hospital admissions and the rates of mortality were analyzed in these groups throughout 18 months (January 2001 to July 2002).

Results

Fourteen patients (30%) from group A were hospitalized, against 24 patients (70%) from group B (p=0.02). Two patients from group A and 1O from group B died (p=0.008). Renal dysfunction was more common among women (31 female vs. 24 ma/e; p=0.008) and in patients over 65 years o/d (41 patients; p=0.001 ).

Conclusion

lt was concluded that slightly a/tered serum creatinine leveis were associated with morbid-mortality among patients with chronic heart failure. Such increases were more common in women and the elderly. The authors high/ighted the prognostic significance of mildly altered serum creatinine leveis in patients with chronic heart failure, despite the estimated creatinine clearance value.
