MOST COMMON DIAGNOSES IN THE ADULT EMERGENCY MEDICAL SERVICE OF A COLLEGE HOSPITAL

Abstract

Objective

To perform an analytical study of the most common diagnoses made at the adult emergency service of a college hospital, as well as to relate them with their respective diagnostic procedures and therapeutic approach.

Methods

ln the year 2001, [at the College Hospital, Brazil] a prospective study of files from the adult emergency service was performed. We compiled information from 729 files, corresponding to a five month service period. The analyzed data included gender, age, origin, diagnosis, case urgency, exams requested, adopted conduct, notes regarding patient's follow-up, and file's legibility

Results

The results revealed that the highest demand of emergency medical care carne from men between the ages of 21 and 40 years o/d. Regarding the diagnoses, the main ones were soft traumas and injuries, followed by general symptoms and respiratory problems. Most of the cases didn't need emergency assistance. The study also revealed that the exams most frequently requested were X ray and blood count. When analyzing the requested exams for diagnoses, we noted that, in cases of soft trauma, the exam mostly requested was X ray; in cases of pneumonia, it was X ray and hemogram; and in myocardical infarction cases, besides the electrocardiogram, the cardiac markers and elethrolytes exams were requested. As regarding the patients' follow-ups, it was noticed that the patients' files did not contain any information on follow-up orientation that might have been given to them. Also, when analyzing the aforementioned files, it was observed that a great amount of them were either partially or completely illegible.

Conclusion

The study indicated that the most common diagnoses were soft traumas, general symptoms, and respiratory i/lnesses. ln addition, the analysis evidenced the

coherence existing between the various diagnoses and the respective diagnostic exams. However, the study revealed that the files lacked registries concerning patients' follow-up orientation; besides, it found a high percentage of illegible files. Therefore, a new type of file was proposed, alongside recommendations for increasing the awareness of the staff involved in the process.
